Webinar Title: Incentivizing Your PAC Network to Meet Your Contract Metrics Date / Time: October 10th 2023 02:00 PM - 03:00 PM (Eastern)
Description:
The demand for post-acute care (PAC) is increasing dramatically and will continue to do so in the decades ahead. As Medicare enrollment expands to nearly 80 million beneficiaries by 2030, 70% will require long-term care and 40% will require skilled nursing care. With post-acute being one of the costliest segments of healthcare, it is essential that ACOs begin developing incentivized strategies to partner with PAC networks in meeting contract metrics. This webinar will explore proven strategies to engage PAC networks in driving care outcomes to meet specific value-based contract goals. Participants will gain insights on how live post-acute data transparency goes beyond ADT and claims data, to deliver the live clinical data needed, at both the patient and facility level, to successfully guide and manage PAC networks. Learn how these key insights will enable ACOs to obtain regular reporting on key clinical and financial metrics, in an effort to develop shared savings opportunities for post-acute partners to drive engagement and performance in meeting contract goals. Afterall, PAC outcomes are your outcomes.
Learning Objectives
Learn how to set and communicate clear and meaningful contract metrics, to drive care outcomes for your post-acute network
Attain PAC buy-in by listening to and understanding what will motivate their performance
Understand how utilizing data transparency, as a partnering opportunity to manage patient care, can result in improved network cost and quality outcomes
Speakers: Phyllis Wojtusik Real Time Medical Systems EVP, Value-Based Care, Kristen Klopp Real Time Medical Systems Network Program Manager

Webinar Title: Interoperability in value-based care: Standardizing information exchange using CORE Operating Rules Date / Time: October 18th 2023 02:30 PM - 03:30 PM (Eastern)
Description:
Attendees will learn about the role Federally mandated and voluntary electronic standards and operating rules play in standardizing and securing the exchange of health information and how they can be leveraged to support the adoption and administration of value-based care (VBC) programs. Joined by Erin Weber, Vice President, CORE and Michael Phillips, Senior Manager, CORE, attendees will understand how CAQH CORE - the HHS-designated Operating Rule Authoring Entity - is updating and creating Operating Rules that:
streamline identification and engagement of patients attributed to a VBC contract,
strengthen collection and sharing of socio-demographic information,
and facilitate the documentation of secondary diagnoses that support risk adjustment and quality measurement methodologies.
Attendees new to the technical interoperability space and those with existing knowledge will equally benefit from the presentation as Erin and Michael detail current and future directions toward automated information exchange.
Speakers: Jennifer Gasperini (NAACOS), Erin Weber (CAQH) [email protected] and Michael Phillips (CAQH).

Webinar Title: Making Care Primary (MCP): Insights for Applicants & the Transition to Accountable Care Date / Time: October 24th 2023 02:00 PM - 03:00 PM (Eastern)
Description:
This summer, the Center for Medicare and Medicaid Innovation (CMMI) announced a new 10-year, multi-state primary care model called Making Care Primary (MCP), which builds on previous primary care models and aims to address population health priorities such as care management for chronic conditions, integrating behavioral health services, and increasing access to coordinated care for rural residents. The application portal is open September 5 through November 30 for eligible practices in participating states. Importantly, the model is intended to attract new entrants to value-based care, with three progressive tracks advancing care delivery and payment reform, and concurrent participation in a Medicare ACO program is prohibited. Join NAACOS for an informative webinar discussing model details and implications for the move to accountable care.

Webinar Title: Navigating the Data Deluge: Transforming Chaos into Order for ACOs Date / Time: October 25th 2023 02:00 PM - 03:00 PM (Eastern)
Description:
In today's healthcare landscape, Accountable Care Organizations (ACOs) face the formidable challenge of managing and leveraging an ever-increasing volume of healthcare data. This data is generated from diverse sources, including electronic health records, claims, patient-generated information, and more. While data holds the potential to drive significant improvements in patient care and operational efficiency, the complexity and sheer volume can often lead to chaos.
This webinar aims to provide ACOs with valuable insights and strategies for taking this data deluge and transforming it into a structured, organized resource that supports the core goals of delivering high-quality, cost-effective care and achieving better patient outcomes.
Key Topics to be Covered:
The Data Landscape for ACOs: Understanding the various data sources and formats that ACOs encounter, including EHRs, claims data, patient-generated data, and social determinants of health.
Challenges and Pain Points: Exploring the common challenges ACOs face in managing healthcare data, such as data intake, data quality, accuracy, and interoperability issues.
The Power of Data Integration: Highlighting the importance of data integration and interoperability in creating a unified, comprehensive, longitudinal patient record.
Real-World Success Story: Showcasing Integrity Health and how they have successfully harnessed data to enhance patient care, reduce costs, and achieve better outcomes.
The Value of Implementing a Data Intake Hub: Discussing strategies for fostering a culture of data-driven decision-making within ACOs, from leadership engagement to workforce training.
Join us for this informative webinar and learn how ACOs can solve healthcare data challenges quickly and easily and unlock the full potential of their data assets to deliver on the promise of value-based care, improve patient outcomes, and achieve operational excellence.

Webinar Title: Unlock your Interoperability and Population Health Initiatives before the 2025 eCQM Deadline Date / Time: October 31st 2023 01:30 PM - 02:30 PM (Eastern)
Description:
Please join us for an opportunity to learn how Lifepoint Health utilizes clinical data for ACO and Population Health goals across the organization, specifically related to value-based care contracts and HEDIS. As the new CMS requirements quickly approach, not only will organizations need to be prepared for submission, but they must also keep interoperability and population health initiatives at the forefront of daily operations.
Starting in PY2021, Lifepoint became one of the first MSSP ACOs to successfully submit MIPS CQMs using the APP API. Their early participation in the program is particularly noteworthy due to the fact that all their ACO entities have multiple TINs and EHRs. In the past year (PY2022), Lifepoint upped the total number of participating ACOs to 8 and submitted a combination of eCQMs and CQMs to maximize their performance.
Speakers: Carol Ann Hudson, RN, AVP, Clinical and Quality Operations, Population Health with Lifepoint Health and Bhushan Kadu, Senior Delivery Manager, ACO PerformancePathway with MRO.
